Everything you need to know about nasal capillary cautery

Medical nasal cautery is performed when it is necessary to permanently seal small blood vessels that break easily and cause bleeding. There are two main approaches.

• In chemical cautery, a chemical substance, most often silver nitrate, is applied directly to the vessel in stick form. At the point of contact, it causes a mild chemical burn, which leads to the formation of a thin protective layer and closure of the capillary. Sometimes trichloroacetic or chromic acid may be used, but only under strictly controlled conditions.
• The second method is electrocautery. This technique uses a special instrument that produces controlled heat. The heat acts on the vessel wall, denatures proteins, and mechanically seals the lumen, preventing further blood loss. This method is recommended for more pronounced and persistent bleeding.

Nasal cautery treatment has the advantage of directly sealing damaged vessels, thereby preventing recurrence. Unlike temporary measures such as packing or drops, this is a long-term solution because it addresses the root cause of the problem. The procedure is quick, minimally uncomfortable, and performed under local anesthesia, allowing patients to return to normal activities rapidly. Thanks to the precision of this nosebleed treatment, damage to surrounding tissue is minimal, and results are reliable.

Symptoms that require visiting a specialist

At Dr Prlja Medical, we recommend nasal capillary cautery when nosebleeds become frequent, prolonged, or do not respond to simple home remedies. You should pay special attention if:

• bleeding lasts longer than 15 minutes and does not stop with pressure or other basic methods
• nosebleeds occur often, from one or both nostrils, interfering with daily life
• dry air or allergies cause fragile vessels to rupture and lead to frequent bleeding
• bleeding appears even after minor injuries, sneezing, or nose blowing
• you notice clots or streaks of blood, even with spontaneous clotting
• the nasal lining looks fragile, with red spots and small sores that open easily

It is important to distinguish anterior from posterior nosebleeds. Anterior bleeds are more common and usually visible from the front, while in posterior bleeding, blood can flow down the throat. This condition requires more urgent examination and intervention.

ENT examination is necessary before starting therapy

Before beginning nasal capillary cautery, a consultation with an ENT specialist and a careful nasal examination are required. The first step is anterior rhinoscopy using a frontal LED light and a nasal speculum. If needed, nasal endoscopy may be performed to examine the nasal cavity in more detail and to precisely locate the source of bleeding. Only after this assessment is it decided which method is safest and most effective for your case – chemical or electrocautery.

The choice between chemical and electrocautery depends on several factors. The chemical method is more often applied in mild and superficial bleeding, especially in children or younger patients. Electrocautery is used when bleeding is heavier, the capillaries are located deeper, or when chemical treatment does not provide sufficient effect. The decision also depends on the patient’s overall health, history of previous nosebleeds, and the ENT specialist’s assessment during examination.

There are also patients for whom the procedure is not performed, such as:

• those with severe facial injuries
• patients with suspected posterior nosebleeds
• individuals with a pacemaker, where additional evaluation is required

Are there risks during nasal capillary cautery?

Possible risks are rare but are always considered:

• Occasionally, minor short-term bleeding or discomfort may occur, usually managed with simple advice and follow-up care.
• In some cases, mild irritation or increased dryness of the nasal lining may appear as the treated tissue heals.
• In patients with more sensitive mucosa, small crusts may form, sometimes causing a sensation of a foreign body in the nose.
• In very rare cases, overly aggressive treatment can lead to thinning of the mucosa or small scar changes, which are monitored during check-ups and corrected with additional therapy.
