Labyrinthitis (Vestibular Neuritis) is an inflammation of the inner ear that disrupts balance and orientation. Although the symptoms appear suddenly, with timely diagnosis and treatment, the condition can be successfully managed. Dr Prlja Medical clinic in Belgrade will explain what the labyrinth is, what causes labyrinthitis, how to recognize it, how long it lasts, and when it’s time to see a doctor.

What is the labyrinth, really?

The labyrinth is a part of the inner ear that plays a key role in maintaining balance and accurate motion perception. It consists of two main parts: the cochlea, responsible for hearing, and the vestibular system, which controls balance.

When this part of the inner ear becomes inflamed, the condition is called labyrinthitis. The inflammation can affect both the hearing and balance sections, so symptoms may include dizziness, nausea, and hearing issues. If only the vestibular part is affected, the condition is referred to as vestibular neuritis.

Labyrinthitis (Vestibular Neuritis) can significantly affect daily functioning, but with proper treatment, the condition usually stabilizes within a few days to a few weeks.

Viral infections like the flu can cause labyrinthitis (Vestibular Neuritis).

Causes of labyrinthitis

Labyrinthitis most often occurs as a result of a viral infection. Viruses that cause the flu or the common cold can spread to the inner ear and lead to inflammation. In rare cases, the cause may be a bacterial infection, especially if there was a prior middle ear infection.

Aside from infections, inflammation can also be triggered by an autoimmune response, where the immune system mistakenly attacks the tissues of the inner ear. Some people are also more sensitive to pressure changes, stress, or physical exhaustion, which may affect symptom onset. It’s important to note that labyrinthitis (vestibular neuritis) is not the same as a middle ear infection—it affects deeper structures and may have more serious consequences if not recognized early.

Possible risk factors

Some people are more prone to developing labyrinthitis, especially during periods when their immune system is weakened. Viral infections like the flu and colds are common triggers. As the body fights the infection, viruses may reach the inner ear.

Stress is another important factor. Prolonged exposure to stress can weaken the immune response and increase the body’s vulnerability to infections. People who frequently suffer from allergies or have chronic inflammation may also be at higher risk.

Low immunity—caused by chronic illness, poor diet, or lack of sleep—further increases the chances of developing this condition. That’s why it’s important to take care of your overall health, especially during virus season.

Symptoms of labyrinthitis

SLabyrinthitis symptoms appear suddenly and can be very intense, especially in the first few days. They mostly affect balance and spatial orientation, and sometimes hearing. Common symptoms include:

Sudden and severe dizziness – a sensation of spinning even when still

Nausea and vomiting – due to disturbance in the vestibular system

Unsteady walking and loss of balance

Blurred vision or trouble focusing

Feeling of pressure in the ear

Mild to moderate hearing loss (in some cases)

Ringing in the ears (less common)

It’s important to distinguish labyrinthitis (vestibular neuritis) from other conditions with similar symptoms. For example, Meniere’s disease also causes dizziness but usually includes ringing in the ears and progressive hearing loss. Labyrinthitis often comes on suddenly and doesn’t always lead to lasting hearing damage.

Sudden dizziness and loss of balance may indicate labyrinthitis (vestibular neuritis) and require medical evaluation.

How long does a labyrinthitis episode last?

A labyrinthitis episode usually begins suddenly and is most intense during the first two to three days. During this time, a person may have serious balance problems and feel the need to lie down constantly.

In most patients, the acute phase passes within a week, while full recovery can take several weeks, depending on the severity of the inflammation and how quickly treatment begins. Some people may continue to experience occasional mild dizziness or a sense of imbalance even after the main symptoms have resolved.

If symptoms persist or worsen, it’s important to see a specialist to rule out other conditions with similar symptoms.

Diagnosis of labyrinthitis

An accurate diagnosis requires an examination by an ENT (ear, nose, and throat) specialist. The doctor will take a detailed medical history and ask about the duration and nature of the symptoms. This is followed by a hearing test, balance assessments, and, if needed, additional diagnostics like audiometry or vestibular testing.

In some cases, an MRI may be necessary to exclude other causes of dizziness, such as tumors or neurological disorders. If you suspect this condition, it’s best to visit a specialist clinic, like our ENT center in Belgrade, for an accurate diagnosis and treatment plan.

Treatment of labyrinthitis

Lečenjreatment depends on the cause and severity of symptoms. If the inflammation is viral, the approach is usually symptomatic—aimed at easing dizziness, nausea, and discomfort while the body fights the infection. Common treatments include:

medications for dizziness (betahistine or dimenhydrinate)

anti-nausea drugs

rest during the acute phase

staying hydrated and getting quality sleep

For more severe or prolonged cases, doctors may prescribe corticosteroids to reduce inflammation or antibiotics if a bacterial infection is suspected. After the acute phase, gradual return to physical activity and vestibular rehabilitation may be recommended.

If dizziness occurs frequently or does not improve, specialized vertigo treatment can speed up recovery and improve your quality of life.

Possible complications if left untreated

If labyrinthitis (vestibular neuritis) is not diagnosed and treated in time, long-term issues may develop. The most common complication is chronic imbalance, meaning the person may continue to feel unsteady or mildly dizzy during quick head movements.

In more severe cases—especially if the inflammation affects the hearing part of the inner ear—there may be permanent hearing loss. This is more common in bacterial infections, which can progress quickly without treatment.

Ignoring the symptoms may also lead to self-misdiagnosis and incorrect use of medications, which delays recovery and increases the risk of further problems.

If you suspect labyrinthitis (vestibular neuritis), see an ENT specialist early for timely treatment.

When to seek professional help?

If you experience sudden dizziness, nausea, balance problems, or changes in hearing, don’t try to diagnose yourself.
